1.     I do hereby give consent to the Lincoln Mali Leadership Foundation and Henley Business School to take photographs and/or videos of me and/or testimonials from me

2.      I also give consent for the Lincoln Mali Leadership Foundation and Henley Business School to use my photographs / videos / testimonials in all publications / prints and marketing material and in all forms of media. The Lincoln Mali Leadership Foundation and Henley Business School are also permitted, but not obligated, to include my name as a credit in connection with the images, videos and/or testimonials. 

3.      This authorisation and consent is given to the Lincoln Mali Leadership Foundation and Henley Business School, without compensation, and I hereby release the Lincoln Mali Leadership Foundation and Henley Business School and/or any legal entity of the the Lincoln Mali Leadership Foundation and Henley Business School from any claims that may arise regarding the use of my images, videos and/or testimonials, in terms of the consent I have given herein, including any claims of defamation, invasion of privacy, or infringement of moral rights, rights of publicity or copyright.

4. I agree that the photos all intellectual property rights of the photos, videos and testimonials belong to the Lincoln Mali Leadership Foundation and Henley Business School.

 5.      Processing of Personal Information:

a.      In accordance with the Protection of Personal Information Act, No. 4 of 2013 (POPIA), and insofar as the Lincoln Mali Leadership Foundation and Henley Business School processes any personal information (including any photographs, video’s and/or testimonials) under or in connection with this consent, I hereby consent to the Lincoln Mali Leadership Foundation and Henley Business School processing my personal information as received by it for the permitted purposes as set out in this consent form, together with such other relevant and applicable purposes as may be set out in Henley Business School's Privacy Notice, located on its website (www.henleysa.ac.za) (the “Permitted Purposes”).

b.      I acknowledge that, by signing this consent form, I have read and understood Henley Business School's Privacy Notice, located on its website. I also confirm that I am over the age of 18 years.

c.      I acknowledge that I have certain rights under POPIA, including (i) the right to access the personal information the Lincoln Mali Leadership Foundation and Henley Business School process about me and to request that such information be rectified, to the extent that it is inaccurate our outdated, and (ii) the right to object to the processing of my personal information, where I feel that my personal information is not being processed in accordance with the consent given in this consent form.

d.      Please refer to Henley Business School's Privacy Policy on its website for further information regarding the rights you have regarding the way that Henley Business School processes your personal information. As mentioned in Henley Business School's Privacy Notice, these rights can be exercised by sending an email, for the attention of the Henley Business School Information Officer, to info@henleysa.ac.za.
